Excel macro with form

The task:
Create a form with a list, data for a file to sub-download from the Excel file, the possibility of the vibration of any values and operations, after pressing the button - get the result

The result:
- data to be downloaded from the Excel list;
You can use any elements of the list.
You can take the operation;
- after pressing the button - red the result in the text field, closed for editing;
- after pressing the "Closing" button the request for confirmation of closure, if no, then reject, if so - closure;
- after pressing the "Vidminity Viber" button cleans the Viber list, operations and result field
