Mailchimp quick application form

Created a landing page for a psychologist on Google Sites. Set up a form for collecting applications for an introductory consultation through Mailchimp: designed the fields, added a checkbox for consent to receive newsletters, connected the form to the Mailchimp audience for automatic contact collection.
