1) Add a status model in the admin panel for Questionnaires:

1. By default, all records have a status of Yes (displayed on the site)
2. The ability for the administrator to change the page to No, then it does not display on the site

2) Add a Source column with parameters User / Import in the admin panel for Questionnaires:

2. By default, all records (pages) have a type of User
3. When adding new pages by the user through the site, Source is set to User
3) Combine the search field into one long field, pre-fill the Placeholder with the text: Enter first name, last name, company name or MC

1. Check that the search works correctly and functions after changes
4) Refine the set of fields for /review/0 (Broker)
Add fields to the page in the front-end:

1. Fields are filled correctly, according to the specifications and data depending on the type of field
2. Data is recorded and saved in the database
3. Extended data is displayed on the site in the admin panel in the card /admin/reviews/*******
4. When adding a comment to the questionnaire, additional data is similarly available:

1. If the user filled out How would you rate this broker *, we display extended information under the main message in the same format (see screenshot below).

now I have a few quesion:
1) Does this status need to be implemented at the database schema level or is it already present and just missing in the admin panel?
2) How is "Import" triggered? Will this happen via future functionality (e.g., CSV import), or is it manually set by admin?
3) Do you want the admin to be able to change Source manually, or is it read-only?
4) Should the search be "fuzzy" across all fields (e.g., partial matches), or do you expect strict matches?
