As I understood, you need to realize a form where each group of fields represents a step.
I can suggest you to create a project on Angular as this framework has a powerful functional to work with forms, field validation, and there are also UI libraries that will allow you to react your form in the form as in your example.
Further, using angular elements we will convert our project into a cast element where you can use the form anywhere on your site/app.
After the project is completed, I will send you the source code with the documentation, where you will be able to make any changes to the functional in the future.
