Filling out 66 medical surveys in Google Forms (Polish language)
I am looking for someone to fill out 100 medical surveys in Google Forms.
Scope of work:
• Filling out 66 Google forms (each taking 4-5 minutes)
• The survey includes questions: gender, medical history, well-being, marital status.
• Most questions are closed (multiple choice)
• Open questions - answers of 1-2 words
• Language: Polish
Requirements:
• Fluent knowledge of Polish
• Carefulness and accuracy
• IMPORTANT: Each survey must be unique (different data: age, parameters, answers)
• Please indicate in the gender question - FEMALE in each survey.
• Data must be realistic and varied. I am attaching a range of correct answers
Conditions:
• Deadline: within 5 days
• Working time: approx. 2-3 hours
Collection and control:
• I will check the uniqueness of the data with the range of answers
• Correctness of filling out all mandatory fields
