Отчёт должен содержать ответы на приведенные вопросы? Или отчёт должен отвечать на другие какие-то вопросы, которых нет в приведенном перечне?
Не понятна фраза "Он может быть в виде панели инструментов, дополненной текстовым полем". Что значит отчёт в виде панели инструментов?
Отчёт должен быть сделан в вордовском файле?
Для подготовки данных для анализа необходимо использовать только формулы или можно пользоваться макросами?

Отчет в том самом документе (то есть Ексель)
Содержит: таблицы, диаграммы, графики.
Может отвечать на приведенные вопросы или содержать свои закономерности, например, между теми и другими данными.
Панель инструментов - неправильный перевод. Правильно - график или диаграмма, дополненная текстом (небольшим, 1-2 предложения, это может быть вывод)
Можно использовать и формулы, и макросы.
