вижу что скрипт не подгружается с шаблона https://rutipo.com.ua/design/rutipo/js/scripts-PageView.js
плюс заранее не показаны какие поля обязательны, только при отправке
возможно нужно ещё доработка, заказ без регистрации
плюс, проверка, чтобы пропускало с тем же email (а то ошибочно пишет что используется уже)
думаю можно будет реализовать
