Если у вас Nginx работает в связке с Apache, и PHP установлен именно в Apache, а Nginx в качестве реверсного прокси, то то в логах Apache все обращения к сайту будут с локального адреса. Если вас интересует статистика по логам, то берите логи от Nginx, а не Apache.
Если важно, чтобы PHP определял адрес посетителя, то тогда используйте переменную $_SERVER[HTTP_X_FORWARDED_FOR] или конструкцию типа:
$user_ip = $_SERVER[HTTP_X_FORWARDED_FOR] ? $_SERVER[HTTP_X_FORWARDED_FOR] : $_SERVER[REMOTE_ADDR];
