1) Поля формы
2) Название формы
3) utm метка с сайтом
4) Источник (Яндекс директ, прямой заход, гугл эдвордс и т.д.)
5) Дата и время отправки заявки
6) Устройства с которого отправлена заявка
7) ip данные (geoip), в том числе страна и город
Указывайте стоимость и срок
Необходимо так же проверять проверку на ввод данных (не отправлять пустую форму), проверять как минимум телефон, после отправки заявки, выводить уведомление "Cпасибо, Ваша заявка отправлена. Мы свяжемся с Вами в ближайшее время")
