Corrections to the form on the Wordpress site

I am looking for a performer who can make edits to the form on the website.
What needs to be done:
There is a form on the website for clients to fill out (I will send you the link in private). To this form, we need to add:
1. T&C text (I will provide the full text)
2. A checkbox with the text "I confirm that I have read, understood and agree to the ....... above". By default, the checkbox is unchecked.
3. An input field for first name and last name.
4. A Sign button.
The button is active only when the checkbox is checked, and the fields contain a first name and last name. After clicking the button, the system sends us an email (for example) or somewhere in the system a PDF document with this data and additionally:
- the time and date of consent
- the IP address
- a unique document number.
