1. Заменить значение поля (скрин формы во вложении). В поле "Номер сертификата" автоматом забивается номер телефона, а необходимо номер сертификата вбить клиенту
2. В данную форму добавить поле "Город", чтоб когда приходит нам письмо на почту, мы могли видеть с какого города поступила заявка
