В том варианте который я думал - анкеты отправляются на необходимый ящик в обычном формате письма. Если надо высылать клиенту то тоже можно сделать в виде письма, в пдф формате не практиковал.

Вам нужно чтобы пользователю отправлялась анкету которую он заполнил или что-то другое ему должно приходить?
Если как бы дубликат того что он заполнил на сайте, то можно сделать отправку письма в виде как захотите вопрос и рядом ответ либо другим удобным образом.

Нет, пользователь заполняет анкету на сайте (визуализация во вложению к проекту).
После заполнения опросника появляетья поп ап "Спасибо за заполненую анкету"... И ответы отправляються владельцу сайта на почту в виде письма или пдф файла

Саму анкету визуально будет сделать не трудно.
А результаты будут отправляться в виде письма таким образом, что владельцу сайта будет понятно на какие вопросы были даны ответы.
