1. Тут чистой воды дизайн (фронт-енд). Создается форма + функционал в виде копирования.
2. После нажатия кнопки - отправить - данные в виде json post запроса попадают на веб-приложение гугл скрипта, в котором вбита таблица. Заполняется скриптом таблица.
3. Печать формы идет из сайта или из таблицы?
