Змінюються лише цифри у кружечках і колір, чи відбувається якась зміна симетрії самої схеми?
Калькулятор має бути окремою web-сторінкою, модальним вікном чи вставленим блоком у вже існуючу сторінку?

Має бути мінімалістичний стиль, як на зображеннях, чи хотіли б бачити щось більш цікаве / "підігнати" під загальний стиль сайту?

Стиль - як простіше. Можна взяти верстку з якогось існуючого сайту

Плагіни - це трохи інше, але зазвичай вони працюють через шорткод, так.
Тут нюанс у тому, що для підключення плагінів потрібна бізнес-підписка. Якщо вона у вас є - то, скоріш за все, це буде кращим варіантом. Але можна зробити і просто через шорткод, зробити сніппет і тд. Тут важко сказати, як краще, не бачивши усі деталі і сам сайт.
Наскільки я розумію, з форми приймається дата народження і повертається значення, що відображатимуться у кружечках (а кружечки змінюють колір, в залежності від значення).
Якщо так, то найкраще, як на мене, зробити ось таке фонове зображення:
а самі кружечки з числами - навіть не через svg, а просто як елементи з фіксованим розміром, border-radius: 50% та id-шками.
Але, знову ж, потрібно побачити сайт, аби остаточно розуміти, що і як реалізовувати.
