Есть форма оригинал

Реализация аналога была не до конца проработана.

В итоге сделано:
- Разметка формы
- Обработчик формы в Firebase с тригерами ( В задании не было предоставлено возможность сделать обработчик на сервере клиента и готового тоже не было)
- Скрипт валидации и масок
При переносе на другой ресурс Форма не работает и показывает в консоле

и форма будет активной до 15 февраля 2021г?
....Я так подозреваю что и окончания после отправки также не будет ...типа "Запрос отправлено"

разумеется у вас мало, что будет отображаться, так как используется плагины для сокращения написания кода. Там есть кнопка Change View(Сменить вид), если нажать, то "вывалится" модальное окно в котором будут видны ваши файлы .html, .css, .js и т.д, чтобы коректно скопировать, нажмите один из них и вас перекинет на страницу с исходным кодом. -
