Эта ошибка появляется тогда, когда открыли какой-то документ, потом его в другом месте программно изменили, и потом в первоначально открытой форме пытаются сохранить. Дальше думайте сами.

может такое быть от того что под одним юзером открыто несколько клиентов с открытой рмк?

Да. Позакрывайте все сеансы. Увидите, как в одном из них выскочит вопрос: "Данные были изменены. Сохранить?"
