Если нужно сделать максимально просто и дёшево то QR код будет вести на форму Гугл - да отзывы не будут приходить на почту, но их всегда можно будет просмотреть в Гугл формах + вы самостоятельно сможете ее подправить по необходимости. На каждого сотрудника можно сделать свою форму. Ну и +дизайн визитки там же не только QR код будет? +знаю как сделать такой QR чтобы после скана телефон сразу записался в телефонную книгу)) вообщем обращайтесь)
