Поставьте меньше время перерегистрации на шлюзе, откройте диапазон портов RTP шлюза на микротике без условий, пробросьте порт 5060 на шлюз со всех ip адресов указанных на сайте Задарма

Если в Mikrotik включен SipAlg отключите и проверьте слышимость.

Согласен с предыдущими комментариями, по описанным симптомам здесь скорее проблема или с потерей Интернет соединения, или с блокировками на микротик. Выложенные же настройки линксисам вполне корректны (правда я из личного опыта предпочитаю в приоритете кодек g711A)
