Основная задача заключается в том, чтобы он мог сделать через смартфон и компьютер подпись. Так как анкета нам нужна с его подписью визуальной.
Можно две анкеты, одна с телефона другая с компьютера, чтобы отправлялась.

Поля для анкеты брал из головы, поставим те которые нужно.
Подпись работает через canvas, как на десктопных так и на мобильных устройствах.
Сейчас картинка с подписью просто сохраняется, в последствии будет отправляться на e-mail вместе с валидированными данными, скрипт на php пишется в течении получаса)

Сделаю без перезагрузки страницы на Angular. Подпись очень просто сделать и вам отправить, можно дать возможность пользователю нарисовать её где-то рядом возле анкеты и потом в виде фото общего с ответами или отдельно отправить вам. Мне потребуется 3-7 дней, всё зависит от дизайна и анкеты.
