на сайте нужно сделать две правки, а именно:
1. не на всех гаджетах отображается видео фон, то есть, на компе норм, на андроиде одном норм, на другом нет, на ISO нет

Покажите пожалуйста сайт, с формой все понятно а вот с видео нет хочу перепроверить, так как если на одном андроиде работает а на другом нет то проблема быстрей всего не с фоном с андроидом или настройками на нем или попросту старая версия андроид, так как не может быть проблема с сайтом что на одном андроиде работает а на другом нет, это проблема с андроидом а не с сайтом. И с ISO проверю.

И на форме просто проставьте float для полей и чуть выравняйте по высоте, и все будет как вам нужно. Работы как таковой нет и платить за нее, даже цену сложить сложно ).
