Необходимо получить все возможные варианты и создать такую же форму.
Готовое решение должно быть в виде плагина для wordpress. Внешний вид можно скопировать с сайта примера. Важно получить все доступные варианты в зависимости от всех параметров.
Форма будет переведена на разные языки, учесть это при написании кода. (gettext)
