С телефонами - не вопрос.С адресами... частично. Если указано, что это улица или проспект. Как обрабатывать "секция, подъезд, поверх, этаж" останется загадкой 🙂
С именами, даже используя библиотеки украинских и русских имён... большой вопрос. Т.к. те же названия улиц - это те же фамилии.

Адрес можно полностью ставить в одну колонку выделить в отдельную только город. Была бы задача простой не писал бы сюда)

Слишком маленькая выборка. Нужен пример больше, чтобы оценить наличие паттернов. Определенные шаблоны просматриваются, но всё же нужен пример больше.

Мне кажется, вам проще попросить реорганизовать вашу базу данных, а для данного типа задания отдельно нанять человека, чтобы он уже разнес существующую инфу по новой БД. 🙂

Есть около 7 таких файлов - человек будет делать такую работу не один месяц. Нет смысла столько ждать.

Телефон из адреса могу вынять самописной функцией(см.влож.),
составить список сокращений и полных названий категорий как бы то же можно и делать подмены при переносе,
а вот отличить ФИО от названия улицы действительно малореально

Реально, ибо есть паттерн ФИО+Телефон или Телефон+Имя 🙂

4 и 5 строка в примере не вписываются в эту структуру. Значит четвёртая часть всех данных не соответствует такому шаблону.
