По поводу иконок - они были бы полезны в PDF который получается в результате заполнения формы и сохранении ее в файл (и выводились бы слева в каждой строке где есть мультивыбор "да/нет").
Если иконки сильно влияют на цену - пожалуйста укажите цену с иконками и без.

Пожалуйста указывайте как вы готовы реализовать форму:
1 в виде pdf файла который можно заполнить и сохранить
2 в виде документа docx, который можно заполнить и экспортировать в пдф (тогда важно что бы работало и под Линукс в LibreOffice)
3 в виде веб страницы, которая отдаст пдф на сохранение (или отправит по имейл). (Это был бы наилучший вариант).
