    Гугл форма всегда привязана к гугл таблице.

    Я думаю надо отказаться от гугл формы. Надо сделать нормальную HTML форму обратной связи. При клике на кнопку ОТПРАВИТЬ данные пересылаются к вам на почту и сразу в гугл таблицу.

    Ну а если хотите внедрить гугл форму, то нужна гугл таблица.

    Создайте таблицу. Потом в таблице сделайте форму. Дайте в таблице доступ по ссылке. И сделайте ее доступной всем.
