Нужно дописать код для отправки с сайта на html и добавить:
* название лида типа "Ленд 315/70 - х шт", то есть слово и часть размера "315/70" из checkbox
* номер тел (ввели на сайте)
* размер (отдельное поле в Б24) - чекбокс с сайта "315/70"
* IP в комментарий
* моб/ПК в источник Лида. ПК по адресу index.php, моб версия phone/index.php, то есть разные странички. Сделать сегодня.

Отправку можно через mail PHP, хотя из за нестабильности данной функции ( не стабильная доставка писем, попадания писем в спам ) я бы рекомендовал phpmailer.
