Есть макет:

Нужно сделать несколько страничек из него:
Верстка адаптивная, при сужении (меньше 980px) меню становится стандартным выезжающим слева (стандартное такое, есть куча скриптов), в шапке логотип и телефон, далее картинка с двумя кнопками (друг под другом), далее контент, и в футере тоже друг под другом поставить нужно будет все.
В исходнике одна страница, но нужно доработать все стандартными элементами, вот список страниц:
1) Главная. Тут все в макете, кроме правых верхних кнопок "найти пациента" и "инструктора"
При клике по первой – попап с этими полями (поля стилизовать стандартными "украшалками" инпутов):
Пол, возраст, диагноз, давность диагноза, адрес, телефон, контактное лицо, галочка "согласен с офертой" (в идеале если она еще в одном попапе будет открываться, если такое возможно, если нет – то просто ссылка), кнопка отправить.
При клике по найти пациента – окно регистраци/входа (разделенного на две част) интруктора с полями:
email, пароль.
В меню "Задать вопрос" – также должна открывать попап с формой "ФИО, Email / Телефон, Ваш вопрос"
Сами отправки форм делать не нужно
2) О сервисе. Такая же страница как главная, но без картинки – баннера.
3) Пациентам. Тоже самое что "о сервисе", но внизу не две кнопки "пациентам" "инструкторам" а одна "Найти инструктора", открывающая такой же попап, как и наверху
4) Инструкторам. Примерно тоже самое, но вместо кнопки будет карта. Ее просто картинкой вставить
