Help needed to fill out these documents

According to part seven of Article 11 of the Law on AML/CFT, the Bank may request, and the client (client's representative) is obliged to submit official documents for verification.

Please provide the following documents and information so that we can continue to serve you:

- documents regarding the determination of your financial status:

1. declaration of assets and income with a stamp of the controlling authority of Ukraine on its receipt (for tax residents of Ukraine); and/or
2. income certificate from the place of employment; and/or
3. certificate from the State Register of Individuals - taxpayers on the amounts of income paid and taxes withheld; and/or
4. other documents confirming the receipt and amount of income;

- description of the nature and economic essence of operations conducted on accounts opened with the Bank, including accounts opened in electronic payment tools (hereinafter - card accounts);

- information on the source of funds: for what purposes you received monetary funds, especially into card accounts, and provide documents confirming the sources of funds;

- information on the subsequent movement of funds: the purpose of transferring monetary funds to accounts of third parties, which are the basis for your cooperation, and provide supporting documents;

- explanation of transactions made on your card accounts involving fund transfers to accounts of individuals (P2P transfers) and asset transactions, including but not limited to, for the purpose of buying cryptocurrencies, funding brokerage or forex accounts, funding e-wallets, purchasing gift certificates, transfers to bookmaking companies, payment of travel checks, etc. In case you made such transfers on behalf and at the request of third parties, please indicate in whose interests they were made.

- explanation of any entrepreneurial or independent professional activities you engage in, and if applicable, please provide:

1. copy of financial statements of the individual entrepreneur submitted at the end of the last reporting period to the relevant authorities, certified with a signature, with a note (if any) of receipt by the state statistical authorities and/or tax authorities, along with the corresponding receipt for submitting the statements in paper or electronic form;
2. business description, including the nature and purpose of activities, market position, presence or absence of a staff of hired employees, main sources and amounts of funds received and subsequent realization of purchased or manufactured goods or services, list of main industrial, commercial, warehouse premises or facilities, logistical support of the business, and other information defining the nature of the activities.

Please provide information (explanations and primary documentation) regarding operations conducted on your account as an individual entrepreneur (explaining the nature of services/works and the availability of operational resources for conducting them to such an extent)

To ensure continued service from the Bank, please provide properly certified documents and information within 10 working days from the date of sending this notification.

Please note that failure to provide these documents or providing false information will result in service denial by terminating business relations in accordance with the first part of Article 15 of the Law on AML/CFT.

According to section 2.22.3 of the Rules for Providing Banking Products and Services to Individuals at Vostok Bank, the Bank has the right to unilaterally terminate the respective Agreement with a notice to the client no less than ten days before the termination of such agreement. This letter serves as such notice.
